Setting up a QMS for MedTech Startups

For new medical device companies, building a Quality Management System (QMS) is often seen as a bureaucratic chore. However, a well-structured QMS is actually a strategic asset that accelerates software releases, simplifies audit preparation, and builds trust with investors.

This guide outlines a lean, digital-first approach to setting up a QMS conforming to **ISO 13485** and **FDA 21 CFR Part 820** guidelines, tailored for modern agile software development teams.

1. Choose a Digital QMS Platform

Avoid paper binders or basic Google Drive setups. They lead to versioning mistakes, lost sign-offs, and audit failures. Start with a digital platform:

  • Dedicated eQMS: Platforms like Greenlight Guru, Qualio, or Matrix Requirements are designed specifically for MedTech compliance.
  • Integrated Wiki (Confluence/Jira): If using Jira, configure plugins like SoftComply to build compliance tracking directly into your ticket workflow.

2. Standard Operating Procedures (SOPs)

A startup does not need the hundreds of SOPs a large medical device manufacturer uses. You can begin with a core set of 5-6 SOPs:

  1. **Design Controls**: How software is planned, specified, coded, and verified.
  2. **Risk Management**: How clinical and technical hazards are evaluated and mitigated (ISO 14971).
  3. **Document Control**: How policies are drafted, reviewed, approved, and released.
  4. **Software Configuration Management**: How branches, builds, and releases are managed.
  5. **CAPA (Corrective and Preventive Action)**: How clinical errors or bugs are investigated and fixed.

3. Establishing a Culture of Compliance

A QMS is only effective if your team actually uses it. Train your engineers on the clinical purpose behind compliance controls. When developers understand that design checks prevent clinical errors, compliance becomes a natural part of the engineering process rather than an afterthought.
